Generate the Official HCA 13-0126
From tier authorization to logged supervision to the official HCA 13-0126 — ready to bill and submit.
Community Behavioral Health Support (CBHS) pays adult family homes for supportive supervision — but only with the right documentation. VentoFlo takes you from the resident’s MCO tier authorization, through logging each supervision interaction, to two finished documents: a detailed internal report and the official, fillable HCA 13-0126 ready for submission. Both are built from the same logged data, with the behaviors flowing from the resident’s finalized Behavior Support Plan.
Step by step
- 1
Set the tier authorization
On the resident’s CBHS card, click “Set Authorization” and enter the tier, authorized hours per day, MCO, and ProviderOne ID from the authorization letter.
- 2
Log each supportive-supervision interaction
Click “Log Supervision,” enter the duration, pick the behavior addressed (its standard interventions appear automatically), and note the trigger and outcome. The minutes feed the monthly billing total.
- 3
Generate the month’s reports
From the monthly section, generate the “Detailed report” (your internal/audit record) and the “Official HCA form” (the fillable HCA 13-0126 for submission).
- 4
Review both documents
Both are produced from the same logged data — the detailed report lists every interaction with times, and the official 13-0126 is filled with the client info, tier, behaviors, daily grid, and weekly summaries, ready to submit.
Why it matters
- CBHS is billable revenue for AFHs, but the HCA 13-0126 and supporting documentation are what make the claim defensible.
- Logging minutes against the authorized tier keeps the monthly total aligned with the tier band — the system flags when it’s below or above so you can correct or request re-tiering before submitting.
- One set of logged interactions produces both the official submission form and a detailed audit record — no double entry, no transcription errors.
