Record a Medication Refusal

A resident declines a dose — chart it as Refused with a reason, in seconds.

Residents have the right to refuse, and a refusal is a documentation event, not a gap in the record. When a resident declines a scheduled dose, VentoFlo lets a caregiver chart it as Refused with the reason attached — so the MAR shows exactly what happened and why.

Step by step

  1. 1

    Open the dose

On the checklist, tap the med’s row to open its actions.

  2. 2

    Choose Refused

Pick “Refused” from the administration options.

  3. 3

    Give a reason

A reason is required — document what the resident said, e.g. “Resident declined — felt nauseous.”

  4. 4

    Confirm

    Tap “Confirm Refused.”

  5. 5

    Done

The dose is charted as Refused, with the reason on the MAR.

Why it matters

  • A charted refusal with a reason turns a missing dose into a documented clinical event — the difference between a records gap and a defensible record.
  • The reason travels with the entry, so patterns (a resident repeatedly refusing a med) are visible to the nurse and prescriber.
  • Requiring a reason keeps refusal documentation consistent across every caregiver and shift.
